My Health Portfolio

We are pleased to introduce My Health Portfolio, a new tool that will change the way you communicate with PCG & SMC. From My Health Portfolio, a new link on our website, you can now complete many tasks without ever placing a phone call to us - at the click of a mouse, 24 hours a day.

My Health Portfolio provides you:

• Access to information contained in your medical record.
  You can track changes in vital signs, view lab results, and verify the dates (and diagnoses) of recent office visits.
• A secure way to communicate with our staff
  Using web messages, complete any of these tasks safely and securely:
• Send messages to (and receive from) our clinical staff.
• Request an appointment.
• Request a prescription refill.
• Report changes in your demographic and/or insurance information.

My Health Portfolio provides patients with an easy-to-use, convenient and secure way to communicate with us. This service is available to established patients of Primary Care Group & Sloan Medical Clinic and requires access to the Internet. To access My Health Portfolio, you must register. My Health Portfolio is only accessible from a link on our website.

My Health Portfolio is currently compatible with Internet Explorer. Other web browsers such as Netscape or Mozila Firefox do not work. Please use Internet Explorer to access My Health Portfolio. For more information, ask a member of our staff today.

Helpful Hints for My Health Portfolio Users

My Health Portfolio should NEVER be used to communicate with us regarding a medical emergency. In the event of an emergency, please dial 911.

If you sent your message to a physician, physician assistant or nurse practitioner, please allow 72 hours for a response. If your message was sent to nursing or non-clinical personnel, please allow 24-48 hours for a response. If you require a faster turn around, please call the office for assistance.

Greeters

Have you noticed a couple of new faces at Primary Care Group? Those faces belong to Greg Segraves and Jack Nolen, our friendly greeters. Greg and Jack are here to greet and assist our patients whether it is to open a door, push a wheelchair, gives directions, or get you a cup of coffee. Our friendly greeters are pleased to help.

Smoke-Free Facility

It is the philosophy of Primary Care Group to provide its patients and employees with an environment that offers the opportunity to optimize their personal health and well-being. In accordance with this philosophy, and the convincing evidence of the negative effects of passive smoke, it is PRIMARY CARE GROUP's intent that our facilities maintain a smoke-free environment. Effective January 1, 2008, PRIMARY CARE GROUP and Sloan Medical Clinic no longer allows smoking on the premises.
